Contract Type: Permanent | Hybrid role | This post is full time for 36.25 hours per week - M-F 9am – 5pm, in winter months we close at 2pm on Friday! (60% home working and 40% out visiting clients)

About the role:

As part of our Social Work and Community Support Service (SWCW), you'll be pivotal in conducting thorough assessments, identifying crucial support areas, and employing a multi-disciplinary methodology to care. Your involvement will extend to spearheading community projects, embodying the values of Jewish Care in various platforms, and enhancing our service and resource offerings. You'll also offer supervision and encouragement to team members and volunteers, fostering a nurturing environment for all.
Key Responsibilities:
Empower and Support: Work hand-in-hand with individuals and carers to tailor care plans that mitigate risks and uplift lives.
Collaborate and Connect: Engage in and promote multi-disciplinary teamwork, expanding networks both within Jewish Care and with external partners.
Community Leadership: Lead and support community and group initiatives aimed at bolstering social and community integration.
Advocate and Educate: Represent Jewish Care across diverse forums, sharing insights on critical areas like Dementia, Mental Health, and more, while providing guidance in line with current policies.
Innovate and Inspire: Collaborate with management to cultivate an impactful service, nurturing relationships across the spectrum of care.
